Operator Members: Individuals employed by a healthcare facility who function as an independent manager of their food and nutrition services (annual dues $195).

• Benefits: Includes all tools free of charge; must pay for on-line education and networking meetings at the member rate. Receive the innovator and HFM BYTES. Eligible to use HFM Member logo. May order printed copy of Successful Operations Guide© at $150.00 (on-line version free of charge). All second and subsequent operators from the same facility at the same location must list the first HFM Operator Member (@$195.00) on application to receive $125.00 rate. Second Operator membership rate only available on HFM web site at www.hfm.org. HFM has agreements with certain Group Purchasing Organizations and/ or Associate Member Companies that offer an annual operator membership rate of $125.00 (sponsored operators). A list of GPO’s and Associate Member Companies supporting sponsored operator dues rates is found on the HFM web site. Only those sponsors listed apply. Sponsored operator membership is available only on the web site or using a membership application provided by one of the program sponsors.
  

Inactive Member: A retired individual formerly employed by a healthcare facility who, when they retired, qualified as either an operator or affiliate member of the Society or whose employment, if any, after retirement is consistent with the goals and objectives of the Society in the promotion of on-staff healthcare foodservice (annual dues $50, lifetime dues $250).

• Benefits: Includes all tools free of charge, except those noted; must pay for on-line education and networking meetings at the member rate. May not purchase benchmarking data reports. Receive the innovator and HFM BYTES. May not receive translator II. May order printed copy of Successful Operations Guide © at $150.00 (on-line version free of charge).
  

Educators: Individuals who are full time members of the teaching facility for accredited schools of Hotel, Restaurants and Institutions, Home Economics, Dietetics, Nutrition or a related field (annual dues $125).

• Benefits: Includes all tools free of charge, except those noted; must pay for on-line education and networking meetings at the member rate. Receive the innovator and HFM BYTES. Eligible to use HFM Member logo. May order printed copy of Successful Operations Guide© at $150.00 (on-line version free of charge) and purchase benchmarking data reports. May not receive Translator II™ or participate in benchmarking.
  

Students: Individuals who are currently enrolled in an accredited foodservice or nutrition program (annual dues $25).

Healthcare Executive Member: Any senior level healthcare executive employed by a healthcare institution that has an on-staff food service department (annual dues $195.00).

• Benefits: Includes all tools free of charge; must pay for on-line education and networking meetings at the member rate. Receive the innovator and HFM BYTES. Eligible to use HFM Member logo. May order printed copy of Successful Operations Guide© at $150.00 (on-line version free of charge). All second and subsequent healthcare executives from the same facility at the same location must list the first HFM Healthcare Executive Member (@$195.00) on application to receive $125.00 rate. Operators from the same facility at the same location must list the first HFM Healthcare Executive Member (@ $195.00) on application to receive the $125.00 rate (all dues are annual, 12 calendar months). Qualifying second healthcare executives and operators from the same facility at the same location may join. However the reduced membership rate for these individuals is available only on the HFM web site (www.hfm.org).
  

Affiliate Member: Any former foodservice manager functioning in an executive capacity, who demonstrates support for non-contract foodservice and who is employed by a healthcare organization that provides direct patient/resident care (annual dues $195.00).

• Benefits: Includes all tools free of charge; must pay for on-line education and networking meetings at the member rate. Receive the innovator and HFM BYTES. Eligible to use HFM Member logo. May order printed copy of Successful Operations Guide© at $150.00 (on-line version free of charge). All second and subsequent healthcare executives from the same facility at the same location must list the HFM Affiliate Member (@$195.00) on application to receive $125.00 rate. Operators at the same facility at the same location must list the HFM Affiliate Member (@ $195.00) on application to receive the $125.00 rate (all dues are annual, 12 calendar months). Qualifying second and subsequent Affiliate Members and operator(s) from the same facility at the same location may join. However, the reduced membership rate for these individuals is available only on the HFM web site (www.hfm.org).
  

Associate Member: Any company or organization which is a food manufacturer, processor, grower, equipment manufacturer, supplier, foodservice distributor, brokerage company, or consultant not in direct operations management supplying independent healthcare foodservice operations.

• Benefits: Two company representatives are included in membership and each receives full benefits. Annual (12 calendar months) dues are $775.00. All HFM tools and services included plus quarterly printed innovator newsletter and monthly HFM BYTES electronic news. Participate at member rates in on-line education and local networking meetings. May purchase advertising (web site, innovator, HFM BYTES); purchase HFM EXPRESS™ benchmarking data; be a Conference as well as HFM program or member service sponsor; participate in Associates Recognition Program; eligible to use HFM Member logo; and may receive HFM member list free of charge. On-line Successful Operations Guide © is included; printed copy available for $150.00.
  

Sponsored Associate Member: Any individual or company who works for a full Associate Member Company and who the Full Associate Member Company sponsors for membership.

• Benefits: A single individual is included in HFM membership and is eligible to receive the following benefits: Annual (12 calendar months) dues are $195.00. All HFM tools and services included plus quarterly printed innovator newsletter and monthly HFM BYTES electronic news.
