Registration Process

Holy Family School

1. All prospective families
complete this form and participate in a school tour

2. After the school tour, start an application on FACTS

3. Schedule assessment with Reverend Martin
Items needed the day of the assessment:

  • Original Birth Certificate
  • Assessment data from Pre-K Teacher for incoming Kindergarten students 
  • Most recent report card for incoming 1st Grade through 8th Grade students 
  • Most recent physical
  • Most recent immunization record
  • Most recent dental screening
  • Most recent vision screening
  • Asthma/Allergy action plan if applicable
  • 504/IEP plan if applicable
  • Incoming coming Pre-K, 1st, 2nd, and 3rd Grade students will also need a hearing screening


4. Schedule a meeting with Business Manager (this can be done while the student is testing). Sandra Nelson 773-265-0550 ext. 505 or [email protected]
One of the following items is needed for the meeting with Business Manager:
  • Tax information for 2023 (first two pages of 1040)
  • W-2
  • SSI declaration
  • IDES determination (Unemployment)
  • Latest 2 consecutive pay check stubs

5.  If accepted, complete enrollment through FACTS

6. Orientation meeting with Reverend Martin

Little Learners Academy Preschool

Registration Process:

  1. You must first complete the Little Learners Academy pre-registration form to begin the enrollment process. You will also receive a copy of the Illinois Action for Children subsidy application at registration if you fall within the guidelines for eligibility.

  2. When there is space available in our program you will be notified by phone to schedule a time to bring your child in for a developmental screening, and advised of all of the paperwork needed as well as any additional enrollment forms.

  3. Prior to your child’s first day of school, you will receive your child’s screening results, and the school's parent handbook. All state required paperwork must be turned in before your child can start school.
